  Capital Cases

  • Appellate Counsel Application  -- for private attorneys willing to accept assignments of OIDS cases on appeal.   Information on this form will be used to make assignments on a case-by-case basis as the need arises in accordance with the experience level and interests of the private attorney. *Application  should be returned to Jessica Adams.
  • The Capital Trial Application  is designed to facilitate the determination of whether an attorney possesses the minimum necessary qualifications under these Guidelines to be included on the rosters of attorneys who are willing to accept appointments as either Lead Counsel or Co-Counsel to represent an indigent defendant at the trial level in capital murder cases.

  Non-Capital Cases

  • Non-Capital Trial Conflict Counsel Representation -  To be approved to provide Conflict representation for the Oklahoma Indigent Defense System in Non-Capital Trial matters, this form must be submitted along with a copy of a letter of Good Standing from the Oklahoma Bar Association and a copy of your Declaration of Liability Insurance. Information on this form will be used to make assignments on a case-by-case basis. Application should be returned to Terry Hervey.
     
  • Non-Capital Trial Overload Counsel Application - To be approved to provide Overload representation for the Oklahoma Indigent Defense System in Non-Capital Trial matters, this form must be submitted along with a copy of a letter of Good Standing from the Oklahoma Bar Association and a copy of your Declaration of Liability Insurance. Information on this form will be used to make assignments on a case-by-case basis. This contract is designed to provide overload services only in counties served by our Satellite Offices.  Counties served: Beckham, Blaine, Creek, Custer, Dewey, Ellis Greer, Harmon, Jackson, Kiowa Okfuskee, Okmulgee, Roger Mills, Tillman, Washita & Woodward. These counties may also be subject to conflict counsel appointments

  Professional Service Providers

  • Expert Fee Schedule - In order to be approved for providing expert services to the Oklahoma Indigent Defense System, this form must be submitted along with a Curriculum Vitae or Resume' and a legible copy of any professional license the applicant may hold.   *Fee Schedule should be returned to Jessica Adams.
  • Investigator Application - In order to be approved for providing investigative services to the Oklahoma Indigent Defense System, this form must be submitted along with current legible copies of professional licenses. In accordance with state statute, all investigators are paid an hourly salary of $30 per hour.
